What sizes do Zepbound vials come in?

Six strengths, one volume: 2.5, 5, 7.5, 10, 12.5 and 15 mg, each supplied as 0.5 mL in a single-dose vial.

The vial is one dose: the whole 0.5 mL is what you draw. Because the volume is fixed and only the milligrams change, the concentration is different for every strength — and the syringe reading is identical for all of them.

Read the right-hand column. Every single-dose strength is 50 units, because the volume never varies — so the unit count tells you nothing about your dose. Six different doses look identical in the barrel. The only thing that distinguishes 2.5 mg from 15 mg is the text on the vial, which is why the label, not the syringe, is what you check. Nearly every page you will find online says a Zepbound vial is 50 units. That is true of the single-dose vial only. The per-dose milligrams in a multi-dose vial are simply the vial total divided by four.

Take a vial labelled 60 mg / 2.4 mL. Divide milligrams by millilitres and the concentration is 25 mg/mL. The intended 0.6 mL dose is therefore 15 mg. Draw the 0.5 mL you are used to from that vial and you get 50 units × 25 mg/mL ÷ 100 = 12.5 mg — about 83% of your prescribed dose, repeated every week until someone notices.

Nothing about that error announces itself. Same vial, same syringe, a familiar number on the barrel, no symptom that says "short dose". The one visible tell is arithmetic: four 0.5 mL draws take 2.0 mL out of a 2.4 mL vial and leave 0.4 mL you cannot account for. If a multi-dose vial is not running out when it should, that is the reason to check.

Why one vial has a preservative and the other does not

The two presentations do not have the same ingredients. The single-dose vial lists sodium chloride, sodium phosphate dibasic heptahydrate and water for injection as its inactive ingredients — no preservative is listed among them. The multi-dose vial additionally lists benzyl alcohol and phenol.

That difference is the whole design. A vial meant to be punctured four times across four weeks needs something to keep it sterile between draws; a vial meant to be punctured once does not. It is also why a single-dose vial is not intended to be part-used or saved for next week — the packaging, not just the labelling, assumes one puncture. Strength is not concentration

"2.5 mg / 0.5 mL" describes a strength in a volume. The concentration is 5 mg/mL, because halving the volume doubles the concentration. Enter 2.5 into the concentration field and the calculator will report a 50-unit draw as 1.25 mg — half the dose actually in the syringe, which could prompt you to draw more of a vial that already held exactly one dose.

Use this to confirm, not to compute a different draw

This calculator runs units to milligrams so you can verify that what is in the syringe matches what you were prescribed. It is not a tool for working out a smaller draw. Zepbound single-dose vials are labelled as one dose and list no preservative among their inactive ingredients, and are supplied as a single dose, so they are not intended to be split across injections or stored part-used. The label also directs that a new syringe and needle be used for each injection. If the number does not match your prescription, stop and call your pharmacy rather than adjusting the volume.

What the label says about the doses themselves

Tirzepatide starts at 2.5 mg once weekly, increases in 2.5 mg steps with at least four weeks at each step, and stops at 15 mg. The recommended maintenance doses are 5, 10 and 15 mg; 7.5 and 12.5 mg are titration steps — the Zepbound titration ladder walks through each rung. The four-week interval matters because tirzepatide has a half-life of roughly 5 to 6 days and reaches steady state at four weeks — a step you judge in week two is judged before the level has settled, which the GLP-1 plotter makes visible. Where this does not apply

Zepbound pens deliver a fixed pre-set dose with no units to read and nothing to convert — see Zepbound pen doses. Is Zepbound available as vials in the UK?
