Missed Dose Guide

Late shot? See what the official labels say, by the hour.

Take it as soon as you can

You're 2 days late, still inside the 4-day window on the Tirzepatide (Mounjaro / Zepbound) label. Take your missed dose now, then go back to your normal weekly day. If moving the day, keep at least 72 hours (3 days) between injections.

These windows come from the FDA labels for weekly injections and can differ for compounded or daily formulations. When in doubt, call your pharmacist or prescriber — this guide is educational, not a substitute for their instructions.

What the FDA labels say about missed doses

The rules differ slightly by medication. For semaglutide (Ozempic/Wegovy), if the missed dose is within 5 days, take it as soon as you remember; if more than 5 days have passed, skip it and take the next dose on your regular day. For tirzepatide (Mounjaro/Zepbound), the window is 4 days (96 hours).

Shifting your injection day

You can move your weekly injection day if needed, as long as there's a minimum gap between doses — at least 48 hours for semaglutide and 72 hours for tirzepatide. That flexibility is handy for travel or scheduling, but keep it consistent once you settle on a new day.

The one thing never to do

Don't double up. Taking two doses close together to “catch up” stacks blood levels and sharply increases nausea, vomiting, and other side effects without any benefit. When a dose is truly outside the window, skipping is the correct — and safe — move.

Frequently asked

What happens if I miss my weekly semaglutide dose?
If it has been 5 days or less since your scheduled dose, take it as soon as you remember. If more than 5 days have passed, skip that dose and take your next one on the regular day. Do not take two doses close together.
How late can I take my tirzepatide (Mounjaro/Zepbound) dose?
Up to 4 days (96 hours) late, per the FDA label. Beyond that, skip the missed dose and resume on your normal weekly day. Keep at least 72 hours between injections if you shift your day.
Can I take two doses to catch up after missing one?
No. Doubling up stacks medication levels and greatly increases the risk of nausea and vomiting with no added benefit. If you are past the missed-dose window, simply skip it.
Does this apply to compounded semaglutide or tirzepatide?
These windows come from the FDA labels for the branded weekly injectables and may differ for compounded or daily formulations. Always confirm missed-dose instructions with the pharmacy or prescriber that provided your medication.
