Switching medications means restarting titration at the new drug's starting dose, not translating your current number. Ozempic vs Wegovy: same molecule, different ceiling

This is the confusion behind a large share of searches. Both contain semaglutide. They are not interchangeable prescriptions.

Ozempic is approved for type 2 diabetes and its ladder stops at 2.0 mg. Wegovy is approved for weight management, includes a 1.7 mg step Ozempic doesn't have, and goes to 2.4 mg — with the new high-dose 7.2 mg option on top.

The practical consequence: you may lose more weight on Wegovy than Ozempic simply because it is prescribed at a higher dose. Trial data reported roughly 15% body weight reduction on 2.4 mg over 68 weeks. That's a dose difference, not a molecule difference.

Wegovy HD: what changed in 2026

The FDA approved a 7.2 mg semaglutide injection — Wegovy HD — in March 2026. Reported trial results were around 21% body weight reduction at 72 weeks, against roughly 15% at 2.4 mg over 68 weeks.

Most dose charts online predate this or omit it. If you're comparing options with your prescriber, it's worth knowing the ceiling has moved.

Mounjaro vs Zepbound: same molecule, different licence

Identical to the Ozempic/Wegovy situation. Same active ingredient, same ladder, different approved indication — Mounjaro for type 2 diabetes, Zepbound for weight management. Which one you're prescribed generally follows your diagnosis and your insurance, not a clinical difference in the drug.

Is there an equivalent dose between semaglutide and tirzepatide?

No. There is no validated dose conversion between them, and anyone who gives you one is guessing.

This is the most-asked and most-badly-answered question in this whole topic, so here is the reasoning.

They are different molecules with different mechanisms. Semaglutide is a GLP-1 receptor agonist. Tirzepatide is a dual agonist, acting at both the GIP and GLP-1 receptors. A milligram of one does not do what a milligram of the other does, and the numbers on the two ladders aren't measuring comparable things — Zepbound's starting dose is 2.5 mg while Wegovy's maximum is 2.4 mg.

The trials were never designed to produce a conversion. Each drug's ladder was established in its own registration programme. No trial mapped "this semaglutide dose equals that tirzepatide dose," so no such mapping exists to cite.

What actually happens when you switch: your prescriber starts you at the new medication's starting dose and re-titrates, regardless of how far up the previous ladder you had climbed. Your tolerability resets too — the adaptation you built on one molecule doesn't fully transfer to the other.

That's frustrating if you were at the top of one ladder. It's also the only safe answer.

Why every ladder waits about four weeks

The four-week interval isn't arbitrary caution. It's pharmacokinetics.

Semaglutide has a half-life of roughly seven days. That means after a dose change, blood levels keep climbing for weeks before flattening at a new steady state — roughly four to five weeks. Tirzepatide behaves similarly, on a slightly shorter half-life.

Two things follow.

You can't judge a dose in the first week. Levels on day 7 are nowhere near where they'll settle. Feeling fine after one injection at a new dose tells you very little about how you'll feel at week four.

Side effects cluster on the rise, not the plateau. The steepest part of the concentration climb is right after a step up, which is why days 2–5 after a dose increase tend to be the hardest and why the same dose feels unremarkable a month later. Our day-by-day side effect timeline maps this out.

Your dose in syringe units

If you're using vials rather than pens, your ladder is in milligrams but your syringe is in units. The conversion:

Units = (dose in mg ÷ vial concentration in mg/mL) × 100

The ×100 reflects a U-100 syringe holding 100 units per millilitre, so 1 unit = 0.01 mL.

Two warnings that matter more than the formula.

Read the concentration off your own label every time. Compounded vial concentrations vary by pharmacy and by batch. The same prescribed dose can be 50 units from one vial and 25 from another. If your vial strength has changed, read switching vial concentrations before drawing anything.

Mounjaro is the dominant weight-management brand in the UK and is available both on the NHS under NICE eligibility criteria and privately through online clinics. Wegovy is also licensed. The ladders are the same molecules on the same schedules; what differs is access, cost and the prescribing pathway.

Staying on schedule

A ladder only works if you're actually on it.
