Every Zepbound single-dose vial contains 0.5 mL of solution, regardless of strength — that's 50 units on a U-100 syringe. A 2.5 mg vial and a 10 mg vial hold exactly the same volume; what differs is how concentrated it is. This is why the mg-to-units conversion everyone searches for doesn't actually apply to branded vials: you draw the whole thing, every time. The answer, and the thing nobody tells you
Let's settle the specification question first, because it has one correct answer.
A Zepbound single-dose vial holds 0.5 mL. Not 0.5 mL for the low strengths and more for the high ones — 0.5 mL for all of them. Lilly formulates each strength so that the full labelled dose occupies the same half-millilitre. On a U-100 insulin syringe, which is graduated as 100 units per 1 mL, 0.5 mL is the 50-unit mark.
Which leads to the part that trips people up.
If you're using a branded Zepbound vial, there is no calculation to do. You draw to the 0.5 mL line — 50 units — and that's your dose, whether you're on 2.5 mg or 15 mg. The number of units never changes as you titrate up. That surprises people, because it feels like a bigger dose should mean drawing more.
If you're using a compounded tirzepatide vial, everything above is irrelevant. Compounded vials come in many concentrations, may be multi-dose, and the units you draw depend entirely on the mg/mL printed on your specific label. This is the group that genuinely needs the arithmetic — and it's the group most likely to get a wrong answer from a page that only describes branded vials.
Almost every article ranking for this question answers for one group and leaves the other guessing. So: branded means 50 units, always. Compounded means it depends, every single time.

Zepbound single-dose vials are supplied in multiple strengths, each in that same 0.5 mL fill.

Look down that last column: it's the same number all the way down. That's the whole point of the single-dose vial design — the dose changes, the draw doesn't.
The mg printed on the vial is the total tirzepatide in the container, not a per-millilitre figure. Reading it as mg/mL is the most common label misread in this category, and it matters enormously the moment you move to compounded product.
How many doses are in one Zepbound vial?
It depends which vial you have, and since early 2026 that question has had two answers.
A single-dose vial holds one dose. That's what "single-dose vial" means, and it isn't marketing language — it's a regulatory category.
A single-dose vial contains no antimicrobial preservative. Preservatives are what make it acceptable to puncture a vial repeatedly over days or weeks; without one, each entry is an opportunity to introduce bacteria into a container that has no defence against them. Single-dose vials are labelled for one withdrawal, for one patient, at one time.
The multi-dose vial — where 50 units is the wrong answer
Zepbound is now also approved in a multi-dose vial, and its numbers are different from everything above. A multi-dose vial holds 2.4 mL — four weekly doses of 0.6 mL each. On a U-100 syringe, 0.6 mL is 60 units, not 50. (Source: FDA Zepbound prescribing information.)
That ten-unit difference is the single most consequential thing on this page. Someone who learned "Zepbound is always 50 units" from a single-dose vial, and then draws 50 units out of a multi-dose vial, has taken about 83% of their prescribed dose — every week, invisibly, with no error message. The syringe looks right. The vial looks similar. Only the label tells you.
The strengths work per-dose, not per-container: a multi-dose vial labelled 40 mg in 2.4 mL delivers four 10 mg doses. Read the dose the label states, not the total in the vial.
So the rule this page opened with needs its exception stated plainly: single-dose vial, 0.5 mL, 50 units. Multi-dose vial, 0.6 mL, 60 units. Compounded vial, do the arithmetic every time. Check which of the three you are holding before you draw, especially the first time a pharmacy changes what it sends you.
The leftover liquid question
You'll draw your 0.5 mL and there will still be liquid in the vial. This is one of the most-searched Zepbound questions, and the answer is mundane.
That residue is manufacturing overfill. Vials are filled slightly above the labelled volume so that a full dose can actually be withdrawn — some liquid inevitably clings to the glass and stays in the needle hub. Without overfill, a portion of patients would end up short.
Overfill is not a bonus dose. It is not a supply of extra medication that Lilly is quietly providing. It's the engineering tolerance that makes the labelled dose deliverable, and the amount is not standardised in a way you could dose from even if sterility weren't a concern — which it is. Re-entering a preservative-free vial to collect residue means puncturing a non-sterile-by-then container with a used needle.
We understand why people ask. Zepbound is expensive and the leftover is visible every single week. But the honest answer is that the residue isn't yours to use, and the risk of drawing it isn't theoretical.
Converting your dose into syringe units
This section is for compounded-vial users, and for anyone cross-checking a dose.
The formula is:
Units = (your dose in mg ÷ vial concentration in mg/mL) × 100
The ×100 is because a U-100 syringe has 100 units per millilitre, so 1 unit = 0.01 mL.
A worked example. Your prescribed dose is 5 mg. Your compounded vial is labelled 10 mg/mL.
- 5 ÷ 10 = 0.5 mL
- 0.5 × 100 = 50 units
Same dose from a 20 mg/mL vial:
- 5 ÷ 20 = 0.25 mL
- 0.25 × 100 = 25 units
Identical prescription. Half the units. That's the entire hazard of compounded vials in two lines, and it's why the number you drew last month tells you nothing about the vial in front of you now.

Drawing from the vial
Your pharmacy's instructions govern here, and the vial Instructions for Use is the authority. In outline: clean the stopper, draw air into the syringe equal to the volume you're taking, inject that air into the vial, invert, withdraw slowly past your target mark, then push back up to the exact line while expelling any bubbles. For a branded vial, pull past the 0.5 mL line and then adjust back to it.
Bubbles matter more than people expect. An air bubble occupying part of your 50 units means you're injecting less than 50 units of liquid.

Branded vials vs compounded vials
These are not the same product and the differences are not cosmetic.
Approval status. Zepbound is FDA-approved. Compounded tirzepatide is not — compounded preparations aren't reviewed for safety, efficacy or manufacturing quality the way approved drugs are.
Concentration. Branded vials are standardised — 0.5 mL per dose from a single-dose vial, 0.6 mL per dose from a multi-dose vial — and the label states which. Compounded vials vary by pharmacy, by batch, and sometimes between refills from the same pharmacy.
Presentation. Compounded vials are frequently multi-dose with a preservative, which changes the sterility picture entirely from a single-dose vial. A branded multi-dose vial is preserved too — but its dose is fixed by the label, where a compounded one's is not.
Regulatory position. This is the part that has changed most, so note the dates. FDA removed tirzepatide from its drug shortage list in December 2024 and semaglutide in February 2025, which closed the shortage-based route that large-scale compounding had relied on. On 30 April 2026 FDA went further, proposing to exclude semaglutide, tirzepatide and liraglutide from the 503B bulks list on the basis that there is no clinical need for outsourcing facilities to compound them.
Patient-specific (503A) compounding has not disappeared, but the bar is a documented clinical reason why the approved product won't do — an excipient allergy, a concentration that isn't commercially available. Cost and preference on their own don't meet it. If you're being supplied compounded tirzepatide today, it's worth knowing on what basis.
